CVS Health Pharmacist - Medication Therapy Counseling (MTM) in WORK FROM HOME, Pennsylvania

This may be the opportunity you've been looking for!The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM). Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health related goals and help them on their path to better health. - Active pharmacist license that is in good standing in state of residence- Must have an active pharmacy license in good standing in a Caremark PBM market (AZ, IL, FL, PA, CA, TX)- Availability for 9:00AM to 5:30PM PT (12:00PM - 9:30PM ET) shift- Availability for rotating Saturday shifts

- Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.- Experience maintaining focus and meeting goals in a work from home environment.- Previous call center experience- Excellent spoken and written communication skills- Basic computer skills and proficiency with Microsoft Office products.- Exceptional prioritization and organizational skills- Acts with integrity and uses sound judgment in dealing with confidential information
Bachelor's degree in pharmacy or Pharm D.
