Job Summary:Interprets medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Prepares compounds and dispenses medications for inpatient and ambulatory care use. This includes the preparation of indictable sterile products. Dispenses Pharmacy related items. Performs duties in all areas of Pharmacy to include inpatient, outpatient, ambulatory care and satellites. Provides information to health care practitioners to promote the rational use of medications. Provides drug-monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy. Interprets policy and procedures for hospital/clinic staff. Participates in teaching programs for hospital/clinic staff. Participates in Pharmacy Departments Quality Assurance Programs. Provides active participation and oversight with transitions of care activities between the inpatient and outpatient setting. As required, coordinates pharmaceutical care in conjunction with the Upstate Outpatient Pharmacy and Specialty Pharmacy Services.

Minimum Qualifications:

B.S. in Pharmacy or Pharm.D., NYS license and registration - or eligible - required; completion of PGY1 Pharmacy residency or equivalent experience required.

Preferred Qualifications:

Previous hospital experience preferred.

Work Days:

8 hour weekday shifts between 8am-6pm, with weekend rotations between 9am-2pm
