Sutter Health Clinical Pharmacist in Sunnyvale, California

Position Overview:

Provides pharmacy services to patients in the infusion center and medical oncology by overseeing treatment plans for accuracy and completeness and ensuring treatment plans are carried out. Recommends adjustments to drug therapy based on clinical status and treatment outcomes. Coordinates with clinical providers to ensure all aspects of pharmaceutical care are delivered in a safe and timely manner. Acts as a resource for other infusion/oncology site(s) as needed.

Qualifications:

REQUIRED: graduate from an accredited school of Pharmacy with a a BS or Pharmacy degree; current California Pharmacy license; minimum of two (2) years Pharmacy experience with background in preparation of IV admixture including strong knowledge of chemotherapy drugs; strong computer skills to interpret and input data into electronic health record, view orders, and maintain accurate inventory; excellent interpersonal, organizational, verbal and written communication skills; strong knowledge of USP 797/800 guidelines; willing (and able) to travel to various PAMF sites as needed
