About the Position
The Clinical Pharmacist is responsible for management of patients in a primary care clinic setting through a collaborative model that includes clinic providers, resource specialists and coordinators. This includes disease state management, patient education, optimization of drug therapy regiments, and other initiatives designed in collaboration with clinic providers and staff.
About the Division
The Community Health Centers have been providing quality primary care and behavioral health services to the citizens of Lane County since 2004. We provide a broad array of services to adults and children in an outpatient setting. The CHC is the primary source for uninsured and underinsured. Our clinics are Federally Qualified Health Centers whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare.
Schedule: This extra help position will provide up to 520 hours of clinical work during the time period of July 2021 to February 2022. Total hours per week will be dependent on candidate’s availability, minimum commitment of 16 hours per week. Potential to work remotely after training period. Clinic hours are Monday – Friday, 8:00am – 5:00pm

Graduation from an accredited School of Pharmacy and registration as a Pharmacist by the Oregon State Board of Pharmacy.
Successful completion of a pharmacy practice residency or ambulatory care residency, OR 5 years of clinical experience in direct patient care delivery outside of retail pharmacy.
One year of pharmacy experience in managed care, disease management, and/or health promotion or equivalent desirable.
Experience in managed care, disease management, corporate wellness and/or health promotion or equivalent desirable.
State of Oregon Pharmacy License
Diploma, CV or Resume
NPI, Current BLS/CPR
One of the essential functions of this position is to travel within Lane County (occasionally outside of Lane County) to completed scheduled meetings.
As part of the screening process Lane County will verify licensure status.
This position is subject to a full criminal offender information record check. If fingerprinting is required, the fee is paid for by the successful candidate and the expense is non-reimbursable.
