Pharmacy Program Manager - PharmacistRemote within OR, WA, ID, UTThe Clinical Pharmacist provides clinical expertise and leadership for the Medication Therapy Management Program for the Federal Employee Program (FEP), working in collaboration with FEP Operations, FEP Directors Office, FEP Members and Network Providers. Responsible for performing member specific interventions through provider collaboration and comprehensive medication review with a focus on cost savings and HEDIS measure gap closure. Identifies, leads, and measures the impact of pharmacy program opportunities that impact benefit cost, utilization and quality metrics.Responsibilities* Collaborate with FEP Operations, FEP Clinical Services and FEP Quality to create clear communication to FEP Members and network providers on medication therapy recommendations and targeted FEP initiatives.* Work in partnership with FEP Director's Office Pharmacy Management Team.* Act as a clinical resource to FEP internal employees regarding medication therapy.* Evaluate medication related problems such as adverse events, therapeutic duplication, adherence to therapy, polypharmacy, and untreated disease or conditions.* Analyzes, documents, and reports member outreach and intervention performance results as required by the FEP Medication Therapy Management Program (MTM Program).* Provide education on the appropriate use of medications to monitor and evaluate patient's response to therapy to FEP clinical staff, members, and network providers.* Participates in effective relationship management with Pharmacy Services' clients that include members, medical directors, case managers, customer service, appeals/grievance panels, and external providers.* Documents all interventions via a medication related action plan or care plan. The care plan is the course of action to achieve the specified health goals.* Support and resolve pharmacy related member specific benefit issues.* Collects, analyzes, interprets, and forecasts health care cost information based on current prescribing patterns based on medication utilization reports.* Collaborate with FEP Clinical Services leadership, including Care Management and Quality, for targeted FEP ongoing initiatives.* Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution.* Utilize client formulary information and member specific benefits to guide appropriate medication recommendations.Minimum Requirements* Strong clinical pharmacy background with the ability to demonstrate clinical therapy skills and knowledge in a clinical care setting.* Strong knowledge and understanding of medication therapy management, pharmacy benefit management, benefit design, formulary management, medication coverage policies, evidence-based medicine, specialty drugs, pharmacy products and services.* Excellent verbal and written communication skills, including the ability to effectively interact with patients, colleagues, and management; strong project management and coordination ability; ability to function effectively as both an individual contributor and part of a team.* Self-motivated, with demonstrated ability to work independently with minimal supervision.* Ability to interpret complex clinical information, claims data and regulatory requirements to deliver quality care to patients and practical business solutions to clients as needed.* Ability to collaborate as part of cross functional teams, to improve clinical programs, enhance processes and share clinical information.* Demonstrated success in managing professional relationships in a managed care systemTo view the full jo
