The Pharmacist Consultant is responsible for supporting disease management programs and physician's care plans through drug therapy evaluation and consultation for program participants.
Primary duties may include, but are not limited to:
Provides pharmaceutical care to members, telephonically, by conducting Comprehensive Medication Reviews (CMRs) for Polypharmacy, drug allergies, clinical effectiveness, cost-effective drug selection, dose optimization, contraindications, adverse effects, potential drug interactions, therapeutic outcomes, and adherence.
Evaluates medication needs and therapies thorough review of medication history to determine if medications are appropriate.
Determines potential barriers to compliance and identifies solutions to overcome barriers.
Ensures that members understand the appropriate use of current prescriptions.
Reviews medications with physicians when therapies do not meet national standards and counsel physicians around alternative medications.
Serves as a resource to the clinical team in disease management and other clinical areas.
Current, unrestricted Pharmacist license in the state of practice.
RPh and minimum 2 years of clinical experience as a licensed pharmacist; or any combination of education and experience, which would provide an equivalent background.
Experience with direct patient counseling or education preferred.
Experience working with members and providers on Medication Therapy Management to optimize medication use required.
MTM Certification a plus.
Ability to multi-task is essential.
Proficient written, oral and interpersonal communication skills required.
Proficient knowledge of Microsoft Windows and Microsoft Office (Outlook, Word, Excel, PowerPoint) required.
This opportunity is for 2nd shift 12pm - 8:30pm. May require response during non-standard hours. Weekend rotations are required.
Call center experience preferred.
