Primary Function/General Purpose of Position

The Clinical Pharmacist Specialist provides comprehensive pharmaceutical care in a specific area of specialty through integrated patient assessment and monitoring, standardization of drug distribution, clinical service, teaching, and pharmacy representation among the medical and nursing staff. Clinical Pharmacist Specialists perform all expectations of the Staff Pharmacists, in addition to expectations specific to Clinical Specialists.

II. Employment Qualifications

  1. B.S. or Pharm. D. from an ACPE-accredited school of pharmacy required

  2. Substantial experience in area of specialty or completion of an ASHP-accredited residency program in area of specialty

  3. Board certification is highly desirable

  4. Licenses: Currently licensed in good standing to practice pharmacy in the state of Virginia

  5. Must maintain competency in unique aspects of drug therapy for patients in area of specialty

  6. 2 -4 years of hospital experience preferred, with strong clinical pharmacy background required

  7. Must demonstrate professional and clinical knowledge of current drug therapy

  8. Ability to provide clinical leadership and education

III. Essential Job Functions

  1. Provides comprehensive pharmaceutical care to patients within specialty area. This includes:

a. Daily review of patients on service (participation in daily patient care rounds with multidiscipline team as applicable).

b. Discussion of medication order clarification with prescribers, documentation of changes in patient and pharmacy records, and communication of changes.

c. Monitoring of drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.

d. Maintaining and updating patient profiles with demographics, diagnosis, allergies, and current medication.

  1. Serves as a source of clinical information for the pharmacy, medical, and other hospital staff. Responsibilities include serving as a preceptor and mentor to Pharmacy Practice Residents, Pharmacy Students, Pharmacists, and other pharmacy staff and providing staff development support to Physicians, Pharmacists, and Nurses.

  2. Ensures safe, appropriate, cost-effective medication therapies to patients in area of specialty. This includes standardization of agents and pharmacy processes. Coordinates policies and procedures for clinical activities regarding area of specialty.

  3. Supports and implements P&T/MEC-approved initiatives by communicating and educating pharmacy, nursing, and medical staff members in service area. P&T assignments impacting service area are to be anticipated.

  4. Participates in hospital-wide and/or corporate clinical, quality, and cost saving initiatives.

  5. Establishes productive, collaborative relationships with medical staff. St. Mary’s Hospital Richmond Community Hospital Memorial Regional Medical Center St. Francis Medical Center Shared Services

  6. Complies formulary stewardship practices. Works with physicians to change prescribing habits.

  7. Sustains professional competence through participation in educational programs, workshops, and national/regional conferences (speaking engagements on area of expertise is encouraged).

  8. Documents clinical interventions according to established procedures.

  9. Prevents, detects, monitors, documents, and reports adverse events and medication errors.
