Position Summary: Osawatomie State Hospital Pharmacy is responsible for providing pharmacy program solutions that reduce costs and improve the quality of patient care. The staff pharmacist provides comprehensive pharmaceutical care through integrated drug distribution and clinical services. The staff pharmacist identifies and resolves medication-related problems to ensure a safe and optimal patient outcome by promoting clinically rational drug therapy and sound pharmaceutical care. In this position you will provide coverage for the pharmacy for illness, vacation, and every fourth weekend.


Job Responsibilities may include but are not limited to the following:


Clinical: Accurately fill prescriptions and dispense medication from the pharmacy. Clarify medication orders with prescriber, document changes, and inform team of changes. Actively collaborate with treatment teams to monitor patient medication therapies that ensure optimal clinical outcomes. Monitor patient medication therapy for incompatibility, contraindications, polypharmacy, and optimal dosing. Convert non-formulary orders to formulary when possible and procure non-formulary drugs when necessary. Review medication orders for appropriateness and check unit dose medications prepared by pharmacy technician.


Operations : Practice in a variety of areas within the pharmacy as required by workload. Ensure compliance with controlled substance distribution and control system. Detect and document adverse drug reactions and medication errors. Deliver medications to treatment staff for patient care if needed. Manage medication inventory to ensure medications are readily available and accessible. Participate in patient care unit inspections as assigned. Serve on hospital committees and task forces as requested. Collect data for department’s quality improvement initiative
