As a Clinical Pharmacist Advanced Practice you will review patient records and medication orders, prepare and distribute pharmaceuticals to patients to provide quality pharmaceutical care directed at appropriate medication therapy and positive outcomes, and assist in clinical rounding on the patients. Some of these activities will be for patients being treated remotely at another Lenox Hill Hospital site
Processes physician orders to provide pharmaceutical care services by screening orders when entering into the computer profile and reviewing for appropriate therapy - e.g., incompatibilities, interactions, dosing, non-formulary agents.
Modifies, clarifies, and verifies medication orders through the interdisciplinary medical team by communicating with practitioners concerning therapy changes
Attends rounds on the unit with the medical staff and assists in drug selection
Assists in Medication Reconciliation with the patient
Assists in codes ( based on area of responsibility such as ED)
Prepares sterile products including large/small volume parenterals, including chemotherapeutic agents as needed.
Dispenses controlled drugs following state and federal regulations and hospital policy including proper documentation and maintenance of inventory levels as well as the dispensing unit for controlled substances.
Maintains proper inventory control by monitoring stock levels to ensure adequate drug supplies to maintain pharmaceutical services and expiration dates to ensure quality control.
Performs computer operations to enter physician orders, maintain patient profiles, generate reports, understand and operate the interfacing of various systems and utilize the dispensing unit for controlled substances.
Performs unit inspection of designated patient care areas by checking controlled drugs, refrigerators, stock levels, medication carts, expiration dating of multi-dose vials, expiration dates, and overall medication handling practices.
Documents activities in the departmental approved data management programs as needed
Reviews and provides analysis of patterns of medication use along with providing recommendations for the department to review
Reviews and evaluates departmental policies and procedures as it pertains to Clinical Pharmaceutical Services on an ongoing basis and submits to the Director of Pharmacy for approval any recommended changes
Ensures review of TJC standards and advises on TJC standards relative to medication use
Monitors nonprofessional staff including the pharmacy students, assistants and messengers.
Attends meetings as required including but not limited to Pharmacy & Therapeutics, and Medication Safety at Lenox Hill Hospital, and Clinical Committees at Northwell Health System as deemed appropriate
PharmD, or advanced ( PharmD/BS)
Minimum of 3 years as a Clinical Coordinator or comparable position in a hospital setting or completion of a PGY1 Pharmacy Residency. PGY2 preferred
ACLS & BCLS & PALS preferred (required in the ED)
BCPS Certified preferred (required within 2 years of hire)
Current license to practice Pharmacy in New York State.
NYS vaccination certification preferred
