As a member of the Pharmacy Department, the Clinical Pharmacist will provide clinical pharmacy services to the PICU. The Clinical Pharmacist will work as part of the multidisciplinary team to deliver safe, appropriate, comprehensive and cost-effective drug therapy.
Job responsibilities include the following:
Participates in multidisciplinary rounds with the healthcare team
Reviews drug therapies and applies appropriate pharmacokinetic and pharmacodynamic principles to provide optimal drug therapy recommendations
Develops and implements clinical practice guidelines to enhance pharmaceutical care and medication safety
Serves as a drug information resource for physicians, nurses, fellow pharmacists, and other healthcare providers
Detects and documents adverse drug reactions, clinical interventions, and medication errors
Doctor of Pharmacy (preferred) or Bachelor’s degree from an accredited college of pharmacy; Current Pharmacist license issued by the Louisiana Boa rd of Pharmacy.
Required: PGY1 Residency or 3+ years of experience
Preferred: PGY2 Residency
Pharmacokinetics, clinical analytical skills. Knowledge of Microsoft Office. Skills in multi-disciplinary team involvement with direct patient care.
