Covenant Health CLINICAL PHARMACIST in Nashua, New Hampshire
Under the supervision of the Pharmacy Supervisor, performs the functions of a licensed pharmacist in preparing and providing medication distribution and drug information throughout the hospital. Monitors the administration of medications to the patient.
Incumbents are identified as "Waste Handler-Satellite Collection Area" employees. These employees must complete the NHDES required hazardous waste level training as defined in the SJH Hazardous Waste Training Plan-2014 as administered by the St. Joseph Hospital Safety Officer.
Review copies of physician's orders for medications, interpret the orders for accuracy and dosage, enter into the pharmacy computer and dispense the required medication.
Monitor patient for drug-drug, drug-food, drug-lab interaction.
Monitor the patient for allergies/potential allergies to medications.
Note any deviations between the medication ordered and that dispensed on the Medication Administration Record.
Monitor the patient's Medication Profile to insure proper administration of medication.
Refill the patient cassettes/Omnicells daily.
Communicate all discrepancies or possible interactions with the medical staff.
Provide drug information to the hospital and medical staffs.
Supervise technicians in all areas of the pharmacy prn.
Provide coverage in the retail pharmacy service.
Perform quality control checks as assigned.
Prepare cytotoxic agents as required.
Conduct monthly security inspections as required.
Inventory and dispense controlled substances as required.
Conform with departmental policies and procedures, hospital directives, safety regulations, and State/Federal Laws.
Must be a graduate of a college of pharmacy accredited by the American Association of Colleges of Pharmacy, or an equivalent association.
Must be licensed by the State of New Hampshire Board of Pharmacy.
