We are looking for a Clinical Pharmacist for the Pharmacy - MV Department at Mission Hospital Regional Medical Center.

Location: Mission Viejo, CA

Work Schedule: On Call / Per Diem

Shift: 10-hour, Days (Variable shift, Evenings)

Job Summary:

Provides pharmaceutical assessment, treatment and care for the neonate, pediatric, adolescent, adult, and geriatric patient populations served. The pharmacist, proficient in the required competencies/skills, is responsible for monitoring drug therapy, providing drug information, consulting with medical, nursing staffs, and others to deliver high quality, cost effective pharmaceutical services to all Medical Center patients. The pharmacist directs the daily activities of pharmacy technicians, interns and students.

Essential Functions:

  • Clinical Activities: Performs clinical pharmacy functions accurately and in a timely manner. These functions include an assessment of all aspects of medication related therapy necessary to provide for the safety, efficacy, and cost-effectiveness of drug use. Clinical activities provided to optimize pharmaceutical care include the regulation of drug therapy; provision of drug information; prevention of drug interactions and adverse drug events; screening for drug allergies; pharmacokinetic, drug level, serum creatinine, potassium, and culture and sensitivity monitoring; evaluation of drug usage for actual and potential medication related problems; intervention to change drug therapy as appropriate; and the reporting and resolution of adverse drug events. Recognizes and addresses the age related needs of the specific population served. Demonstrates the ability to assess/meet the psychosocial and developmental age related needs of the specific population served. Consults with medical and other staff regarding drug therapy. Provides consultation with patients/families on medication use as appropriate. Participates in the pharmacy QI/DUE programs by collecting and evaluating data as required. Responds appropriately to emergency situations. Assists the Director in implementing and administering plans and programs, including committee participation. Utilizes good communication skills in handling all issues. Demonstrates ability to perform clinical competencies as indicated on the competency/skills inventory.

  • Distribution Activities: Processes and accurately interprets all medication orders in a timely manner. Reviews all orders for appropriateness and accuracy, questions incorrect or incomplete orders, contacts physician and recommends therapeutic alternatives to non-formulary drugs, and performs the drug distribution procedures necessary to ensure that the right drug is dispensed to the right patient at the right time. Fills orders for all medications, including IV admixtures, TPNs, chemotherapy, compounded formulations, re-labeling of outpatient prescriptions, and investigational drugs in a timely manner. Demonstrates ability to assess/meet the developmental age-related needs of the specific population served. Work area is kept neat and orderly. Is aware of and follows all applicable policies and procedures in dispensing and compounding medications. Completes all required paperwork accurately and in a thorough, timely manner. Assures compliance with all department and hospital policies, as well as, state and federal regulations. Utilizes good communications skills in handling all issues. Demonstrates the ability to perform the distributional competencies as indicated on the competency/skills inventory.

  • Technician Supervision and Training Activities: Directs and closely reviews the daily work of pharmacy technicians, interns, and students following established procedures. Provides guidance and direction in administration of duties. Utilizes good communication skills in handling all issues. Assures all work of the technicians is accurate and thorough. Assures aseptic techniques are followed. Continually assesses and prioritizes work load to ensure the proper operation of the pharmacy using effective supervisory, communication, and problem solving skills. Responsible for providing staffing adjustments for unexpected workload changes or staffing problems. Assists in the orienting and training new technicians, interns, student, or other hospital employees as necessary by demonstrating techniques and explaining procedures. Provides feedback on annual technician evaluations in a complete and timely manner.

  • Inventory Control: Accurately assesses and communicates the need for restocking in a timely manner. Responsible for medication inventory, storage, and distribution. Maintains accurate narcotic, investigational drug, and manufacturing records according to established procedures.

  • Education Activities: Responsible for the self-directed education necessary to develop, maintain and improve professional competence. Commits to improving knowledge of current trends and developments in the medical and pharmaceutical field by reading appropriate journals, books, and other literature and by attending seminars and conferences. The pharmacist’s Continuing Education Log reflects the age specific needs of our patient population. Completes competency tests as required by the department. Provides accurate up-to-date drug-related information to the medical, nursing, and pharmacy staff and the community. Participates, as requested, in in-services and C.E. programs, and serves as a preceptor to 4th year pharmacy students.

Skills:

  • Interpersonal skills, as well as, verbal and written communications skills necessary to communicate effectively with physicians, nurses, students, pharmacy personnel, patients, administrative, and other hospital personnel.

  • Thorough knowledge of neonatal, pediatric, adult, and geriatric institutional pharmacy operating systems; California and federal pharmaceutical law; and JCAHO and Title 22 requirements.

  • Analytical skills necessary to determine the appropriateness of drug therapy and the correctness of the dosage, and be able to evaluate drug interactions, adverse drug effects and allergic reactions.

  • Organizational skills necessary to coordinate all drug distribution and clinical activities.

  • Consistently demonstrate an ability to keep up with the patient workload: responding to changes in the workload as necessary to complete the job while coordinating the activities of pharmacy technicians, interns, or students.

  • High degree of independent problem resolution skills to effectively solve pharmacy related operational and clinical problems.

  • Data entry skills.

  • Working knowledge of pharmacokinetics, pharmacology, and toxicology as they relate to drug therapy.

  • Ability to concentrate and pay close attention to detail when reading and interpreting physician orders and related materials, and examining and closely inspecting materials when filling medication orders and preparing or checking IV solutions.

  • Physical ability to walk and/or stand during up to eighty-five percent of work shift and be able to lift boxes of pharmaceutical supplies up to approximately 30 pounds.

  • Proficient in required competencies upon completion of orientation, as listed in the Pharmacy competency/skills inventory.

Minimum Position Requirements:

Experience: Clinical pharmacy experience in an acute care hospital setting.

Licenses/Certifications:

  • Current California license as a registered Pharmacist.

  • Current American Heart Association BLS Healthcare Provider CPR card.

Preferred Position Qualifications:

Education: PharmD and completion of a hospital pharmacy residency or Clinical experience in a comparable acute care hospital.

Licenses/Certifications: Current American Heart Association ACLS provider card.
