Pharmacist Clinical Coordinator - PHARMACY contributes to AHF’s mission by improving the clinical outcomes of our AHF Pharmacy patients through Medication Therapy Management (MTM). Pharmacist Clinical Coordinator provides a helpful voice, to our patients, clients and customers seeking MTM services, clinical information, and support for their medication adherence.
Providing a high level of clinical skills and knowledge to our patients that helps them live a sustainable quality of life, be productive in society, and improving their health contributes to the success of AHF and our mission. This also contributes to society in general.
Within their duties of Medication Therapy Management, they are problem solvers, providing a “helping hand” and provide a calming influence to patients ensuring they understand everything about their medication including: What is it treating? How do I take it? What are the side effects? This PHARMACIST also reviews for medication problems, monitors medication adherence, reviews immunization history, and provides interventions with patient and Primary Care Physician when necessary for improved pharmaceutical care.
The Pharmacist Clinical Coordinator - PHARMACY can enhance the experience of other AHF Pharmacy employees by supporting the Clinical Pharmacy operations in the home pharmacies throughout the country. Providing a helping hand in ensuring the Clinical Patient
Management Program runs smoothly in our home pharmacies helps to further improve our patient’s health outcomes and contributes to the ultimate mission of AHF Pharmacy.
The Pharmacist Clinical Coordinator – PHARMACY will work with the clinical team and home pharmacies to maintain the standards of specialty pharmacy accreditation. This will include but not limited to providing clinical training updates for the AHF Pharmacy staff, updating patient management program documentation platforms and assisting with patient’s clinical assessments.
The Pharmacist Clinical Coordinator- PHARMACY may be a preceptor to pharmacy students completing their APPE rotations.

Additional contributions such as assisting in the development and implementation of new systems and procedures, becoming “HIV Knowledgeable”, becoming cross trained in all aspects of the pharmacy operations and accepting assignments on a rotating basis, are all important contributions to our success.
Performing as a supervisor in the absence of the Pharmacist In-Charge/Manager, is essential in maintaining a consistent level of service expected by our patients and the success of the operation of the pharmacy.
Assisting AHF in providing “gold standard” customer service, becoming certified when appropriate, based on contracts, grants, Memorandum of Understanding agreements or any other Ryan White or Accreditation Standards, related to training, contributes to the growth of AHF. In addition, enrollment of new patients into service and the retention of patients in care, as well as being aware of competitor’s, their services, and effectiveness in the market place are contributions that are at the core of our success.
Contributing to the success of patients and clients with the improvement of their quality of life by participating in various programs and services that enhance our patients’ adherence to therapy and treatment, communication with providers and referral sources.
Thorough and timely documentation of patient encounters that provides a continuity of care between other AHF Pharmacy team members and providers, assures that patient care is not comprised or diminished from one team member to another.
Becoming familiar with all aspects of AHF’s mission, core values and scope of services, participating in community activities of AHF, AHF Pharmacy, AHF Affiliates, Affinity Groups and AHF Departments contributes by promoting the mission of AHF and the spirit of “giving back” to the communities we service.
EXPERIENCE, EDUCATION, LICENSES AND ABILITIES THAT ARE ESSENTIAL CONTRIBUTIONS
Success for the position of a Pharmacist Clinical Coordinator requires that the team member have a current Pharmacist license (in the State where employed) and/or hold a American Academy of HIV Physicians Certification for Pharmacist (AAHIVP, current or successfully obtain within 12 months of employment), have data input skills and experience in pharmacy systems, which includes typing skills and familiarity with Windows PC applications.
Must have language skills that include the ability to read and interpret prescription documents, policies, procedures and guidelines, interpret documents such as safety rules, operating and maintenance instructions, write routine reports and correspondence, including entries in various computer programs utilized by AHF and AHF Pharmacy, must have the ability to speak effectively with patients, clients and staff members in English (Bi-lingual in English and Spanish highly desirable and Creole in some specific geographies).
Exceptional phone etiquette and customer service is an absolute must. Phone interpersonal skills are imperative to ensure the MTM encounter with the patient is a success. Knowledge of motivational interviewing techniques is also valuable to engage the patient in the process and to collect the information necessary for a successful MTM encounter.
Must have at least intermediate skills to calculate figures and amounts such as a day’s supply of medications based on the prescribed directions, the ability to calculate dispensing and billing cycles based on days’ supply and billing dates, must have the ability to calculate figures and amounts such as discounts, volume, percentages related to prescription charges and selling prices of goods and services proved by AHF Pharmacy.
Reasoning Skills contribute to patient satisfaction and wellbeing by solving practical problems and dealing with a variety of concrete variables in situations where only limited standardization exists, as well as the ability to interpret a variety of instructions furnished in written, oral, diagram, or scheduled format.
