Leads antimicrobial stewardship strategy for accountable site(s) in collaboration with system antimicrobial stewardship team.2. Assists in fulfillment of team member staffing vacancies (e.g., TAP) when necessary.3. Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients receiving antimicrobials.4. Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in infectious disease through improved reporting, analysis and follow-up.5. Coordinates and maintains the antimicrobial stewardship program, including:a. Overseeing utilization audits of antimicrobial therapyb. Assisting in development and maintenance of an antimicrobial formulary approved by the P&T Committee.c. Working with the infectious disease service and hospital medical staff to evaluate appropriate use of restricted antimicrobialsd. Developing guidelines, policies, electronic health management solutions and other monitoring and intervention strategies for antimicrobial use and streamlining prophylactic, empiric, and definitive antimicrobial therapye. Reviewing data and metrics on antimicrobial usef. Reporting data and findings to the Infectious Disease Clinical Guidance Council, and Pharmacy & Therapeutics Committees.g. Coordinating, facilitating and promoting education, training, competency assessment and performance improvement of pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antimicrobials6. Serves as pharmacy consultant to Infectious Disease consult service7. Precepts pharmacy residents and pharmacy school students in Infectious Disease rotations.8. Responsible for ensuring compliance with evidence-based recommendations and regulatory standards related to safe and appropriate antimicrobial use.Staffing ResponsibilitiesThis position will work every 4th weekend. Optimal candidates will be PGY1 trained with focused experiences in infectious disease and/or PGY2 trained in infectious disease. BCPS or BCID preferred.Performs duties expected of a staff pharmacist. During weekend staffing, pharmacists are responsible for dispensing and compounding medication, counseling patients on medication and health subjects; interpreting prescriptions, managing patient drug therapy for complications, and performing drug utilization. Pharmacists will be expected to cover emergency responses (e.g., Code Blues, Trauma alerts to the E.D.). He/She collaborates with physicians, nurses, and other health-care practitioners to provide safe, high quality, and cost-effective drug therapy.Pharmacists may be required to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise..Minimum Qualifications:Doctor of Pharmacy (Required)OPL - Ohio Pharmacists License - Ohio Board of PharmacyBS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license.
