Elliot Health System CLINICAL PHARMACIST in Manchester, New Hampshire

40 hours per week, 3:00pm-11:00pm

  • POSITION SUMMARY

  • Clinical Pharmacists work independently within the limits of established pharmacy practice and within the scope of departmental policies and procedures as a member of the patient care team.

  • KEY RESPONSIBILITIES

  • Medication Orders: Process medication orders entered in provider-order-entry by the prescribers for promptly and accurately providing medications for administration to patients.

  • Patient Specific Information: Monitors drug therapy to prevent contraindications, duplicate therapy, drug interactions and allergic reactions. Recommends appropriate medication doses and adjustments as indicated by patient conditions. Responds to drug information requests with accurate, clinically relevant, reliable and valuable information in a timely manner.

  • Education: Provides training and in service education to pharmacists, nurses, providers, students and residents as applicable.

  • Development: Actively involved with personal professional development and growth to support clinical care of patients. Shows engagement with and completion of education and training beyond licensure requirements. Takes action to improve work and departmental performance.

  • Formulary: Understands hospital formulary and adheres to hospital formulary whenever possible and provides recommendations for therapeutic interchange as necessary. Participates in medication utilization or other assessments of clinical, cost effective use of medications.

  • Oversight of Pharmacy Technicians: Checks technician work for accuracy. Understand, supports and organizes technician work flows to meet patient needs and improve patient care. Provides leadership and direction in oversight responsibility with technicians.

  • Quantity of work: Works efficiently to complete assignments and meet deadlines. Executes workload efficiently and within time alotted and uses any downtime toward clinical projects and departmental goals. Performs additional duties as defined by leadership based on department needs.

  • Quality of work: Work is consistent, complete, accurate and documented professionally. Takes action to improve work and departmental performance.

  • Attitude/Teamwork: Works effectively and positively with all interactions. Is a team player. Treats coworkers with respect in all conversations. Participates in team meetings and recognizes team efforts with the group.

  • Adaptability: Adapts to change in a constructive manner. Is able to balance changing priorities and varying tasks and demands. manages stress levels appropriately.

  • REQUIRED KNOWLEDGE & SKILLS

  • Must have excellent oral, written and interpersonal communication skills to communicate effectively and work collaboratively with members of the healthcare team.

  • Must understand the basic operations of the pharmacy department including equipment/technology in use and be able to address issues in real time.

  • Prioritize and organize work while under stress and with time constraints. Must be detail oriented and possess critical thinking and decision-making skills.

  • Must maintain sufficient knowledge of pharmacotherapy for the safe and effective support of medication processes.

  • Must possess leadership abilities in clinical pharmacy practice.

  • EDUCATION/EXPERIENCE/LICENSURE

  • Education: Professional Pharmacy Bachelors Degree required. Pharm D Degree preferred.

  • Experience: One (1) year previous pharmacy experience preferred.

  • Certification/Licensure: Current New Hampshire Pharmacist license to practice Pharmacy required. Basic Life Support Certification preferred but not required.

  • Software/Hardware: Epic Willow Pharmacy System; McKesson Automation Products experience preferred.

  • OTHER REQUIREMENTS

  • Schedule: Varying shifts based on department needs.
