UCLA Health Medication Reconciliation Pharmacist in Los Angeles, California
Under supervision, the Medication Reconciliation
Pharmacist, when assigned to a specific patient population, shall be
responsible for the medication reconciliation process at the patient's
admission to prevent medication errors at this patient transition point. The
pharmacist shall, by interviewing patients, create a complete and accurate list
of all pre-admission medications which the pharmacist shall compare against the
physician's admission orders to identify and bring any discrepancies to the
attention of the physician and, if appropriate, make changes to the orders with
the physician's concurrence.Qualifications
CA pharmacist licensure or eligible for CA pharmacist licensure.
Doctorate of Pharmacy Degree from an ACPE-accredited College of Pharmacy.
2 years of inpatient hospital experience or PGY1 residency
Completion of PGY2 Pharmacy residency
Experience in inpatient medication reconciliation
