UPMC Clinical Pharmacist - Ambulatory in Lemoyne, Pennsylvania
Ensure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research.
Collaborating with Primary Care and Specialty practices, the Ambulatory Clinical Pharmacist will design, evaluate and modify pharmacotherapy for specific populations in ambulatory care.
Implements protocols to enhance pharmaceutical care services for patients served by the Medical Group practices.
Strives to create meaningful, interactive relationships within the patient-centered medical home and help to achieve high quality health outcomes for populations served.
This role will support quality improvement and resource management initiatives, while participating in scholarly activities and/or clinical research in ambulatory care.
Opportunity to educate health professionals and patients regarding optimal medication therapy.
Complete Medication Therapy Management sessions one-on-one with patient populations, either through telephonic on in-person sessions.
Pharm.D required. B.S. Pharmacy Required.
Pennsylvania state license required.
Must have excellent interpersonal, verbal, and written communication skills.
Very high level of independent judgment and problem-solving skills.
Strong attention to detail.
Record of effective working relationships with other health professionals.
Excellent oral and written communication skills.

Demonstrate success in relationship building.
Preference: 3-5 years ambulatory pharmacy experience or completion of PGY2 ambulatory care residency.
Licensure, Certifications, and Clearances:
Clearances must be dated within 90 days

Act 73 FBI Clearance with renewal
