Trinity Health Inpatient Pharmacist, Per Diem/Pool (St. Mary Medical) in Langhorne, Pennsylvania

Description:This pharmacist will be responsible for all medication verification/distribution and clinical activities. The candidate should also have knowledge of automated medication distribution systems, USP797 and the ability to prepare intravenous medication utilizing aseptic technique.

The successful candidate should possess good communication skills, including the ability to interact successfully with other colleagues to ensure efficient and safe distribution of medications. Ability to promote teamwork is essential. There is a weekend and holiday commitment with this position and the opportunity to pick up additional shifts.

  • Demonstrates proficiency with, and understanding of the medication distribution model including but not limited to Pyxis, MedCarousel and MedBoard.

  • Reviews medication orders for appropriate dose, utilization, adverse reactions, allergies and drug interactions. Bill and dispense medications in accordance to pharmacy policy.

  • Clarifies problem medication orders prior to dispensing medications. Clarifies orders directly with physician, notifies nurse of pending issues and communicates pending issues during handoff communication with other pharmacists.

  • Demonstrates proficiency in and actively participates in department’s clinical initiatives. Including but not limited to pharmacy managed warfarin protocol, pharmacy managed vancomycin protocol, IV to PO therapeutic interchanges, epogen protocol, heparin Protocol and antimicrobial stewardship activities such as Biofire.

  • Monitor the clinical surveillance platform (Sentri7) daily and documents clinical interventions in the intervention tracking software (Quantifi).

  • Reviews medication orders for safety, accuracy and clinical appropriateness. This includes but is not limited to monitoring drug levels, monitoring renal function, reviewing interactions (drug-drug, drug-supplement, drug-food) and reviewing antimicrobial therapy.

  • Communicates appropriately and timely with prescribers regarding recommendations for medication dose adjustments, antimicrobial therapy (indication, medication selection, duration of therapy) and to discuss any other medication related recommendation.

  • Routinely provides physicians, nurses and patients with information regarding safe and effective use of medications.

  • Communicates with all patients (and their families) to obtain necessary information, address patient/family concerns/questions. Provide counseling and education to patients on inpatient and discharge pharmacological therapy.

  • Distributes time among all assigned patient care units as decentralized pharmacist. Rotates among all assigned units throughout entire shift.

  • Maintains current knowledge and techniques necessary to provide pharmacy services of the highest standards.

  • Observes and checks work performed by the pharmacy technicians throughout the medication preparation and dispensing process as appropriate.

  • Complete stock rotation by expiration date to ensure medications accessed are within date; this includes any medications stocked in Pyxis, MedCarousel and any pharmacy locations.

  • Rapid response boxes, intubation boxes and code carts are filled accurately and with proper amounts of medication and expiration dates in a timely manner.

  • Communicates low stock or increased usage of medications to the Buyer to ensure appropriate supplies are available or on order.

  • Works effectively with nursing and pharmacy colleagues to assure safe and accurate distribution of medications via automated dispensing cabinets.

  • Participates in main pharmacy workflows by answering telephones and relays messages when appropriate, running stat medication deliveries or completing other tasks when needed.

  • Demonstrates proper disposal of intravenous products and expired medications. Including compliance with waste of hazardous medications in line with Stericycle policies and procedures.

  • Actively participates in hand-off communication with next shift.

  • Assists in the training and orientation of new colleagues.

  • Performs all other duties as assigned.

    PA Licensed Registered Pharmacist
