Summary

Job title: Pharmacist Full-time 1.0 FTE

This position is located in Lancaster, PA

Full-time, Hours: 80 hours per 2 weeks M-F with varied shifts (0600-1430, 0830-1730, 1200-2030, etc)

On call evenings: 4 evenings in an 8 week cycle (M ,T, W, Th 1700-0830 next day)

Weekend: On call every 8th weekend (Friday 1700 - Monday 0830)

Holiday: Every 8th Holiday

**On call/Weekend/Holiday rotations subject to change based on staffing levels. Located at Horizon Healthcare Services.

POSITION SUMMARY:

Provides pharmacist services in accordance with the rules and regulations of the Pennsylvania State Board of Pharmacy, the U.S. Drug Enforcement Agency, other regulatory bodies and the Policies and Procedures of Horizon Healthcare Services.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

Dispenses Medications:

Accurately compounds, labels and dispenses medications per physician orders utilizing appropriate techniques and completing proper documentation (including lot numbers and expiration dating information) within established timeframes.

Considers factors such as IV access, patient/caregiver ability, and fiscal limitations and determines the most appropriate method of medication administration.

Interprets and evaluates physician medication and IV admixture orders, obtaining clarification as deemed necessary, and carefully reviews the prescription, retrospectively and prospectively, to prevent adverse reactions, allergic reactions, drug interactions and improper dosing. Consistently recognizes inappropriate physician orders and takes action to resolve problems without resulting delays in patient treatment. Documents findings and interventions appropriately.

Controls the disposition of narcotics and other scheduled drugs as required by State and Federal regulations. Assists with inventory control for pharmaceutical products and accessories.

Responds to drug information requests as required, effectively utilizing library and reference sources. Accurately & effectively counsels patients on the appropriate use of medications & responds to drug information requests in a helpful & professional manner. Provides patients with the basic information necessary to safely administer medications.

Demonstrates a thorough knowledge of all software systems used to complete pharmacy functions and shares knowledge freely with other staff.

Thoroughly reviews all products made by a pharmacy technician for accuracy and correctness & resolves problems prior to products being dispensed.

Ensures sterility of medications by consistently following established policies for gowning, gloving, and aseptic technique procedures when working in and maintaining a clean room environment.

Always complies with federal and state laws/regulations regarding pharmacy practice.

Monitors Clinical Therapy:

Consistently follows established policies regarding patient assessments and re-assessments.

Demonstrates a thorough knowledge of the care planning process. Initiates appropriate care plans at the start of therapy. Through communication with patients, nursing staff and physicians, monitors medication effectiveness and makes recommendations for therapy changes as appropriate, and completes and updates the Pharmaceutical Care Plan as required.

Reviews labwork in a timely manner and responds to abnormal lab values by making appropriate recommendations to the prescribing physician.

Collaborates with nursing staff to ensure that care provided is appropriate, effective, and meets goals established in the care plan.

Communicates any breaches or deficiencies in processes or organization performance which place patients, employees or the organization at risk to the attention of the Pharmacy Supervisor, Pharmacy Manager and President.

Quality Improvement:

Actively participates in the CQI program, making recommendations for process improvement and identifying processes requiring intensive examination and study.

Demonstrates a thorough understanding of JC standards and acts to ensure compliance with all of the standards.

Participates in pharmacy policy development, demonstrating a complete knowledge of pharmacy policy and making recommendations for improvement.

Participates in the evaluation of new drugs and the development of new programs and makes recommendations to the Manager-Pharmacy Services.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

Ensures proper operation of equipment and assists with preventive maintenance of the clean room and equipment as directed.

Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

B.S. degree in Pharmacy, or PharmD.

Current, valid Pennsylvania Pharmacist license in the Commonwealth of Pennsylvania (or eligible to reciprocate)

Familiarity with computer-based operating and reporting systems and knowledge of network computer usage (or ability and willingness to be trained.)

Compliance with federal, state and other funding agents: A healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e., not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.

PREFERRED QUALIFICATIONS:

Minimum of two (2) years’ experience as a pharmacist preferred.

Hospital or home-infusion pharmacy experience preferred.

This position requires a license.

This position requires FBI and Child Abuse clearances
