JOB SUMMARY Performs all functions of pharmaceutical care including medication and IV orders. Evaluates for appropriate drug, dose, allergies, interactions, and incompatibilities. Monitors levels of therapeutic drugs. Performs and supervises computer data entry related to patient profile. Monitors all medication dispensing for accuracy. Supervises filling of all prescriptions and provides appropriate patient counseling. Collects and analyzes data for process improvement projects. Performs in-service to community and support groups. Performs other duties as assigned. May be required to work alternate shifts. Performs other duties as assigned. ESSENTIAL DUTIES AND RESPONSIBILITIES ? EDUCATION, TRAINING, & EXPERIENCE ? Graduate of an accredited Pharmacy Program ? Hospital Pharmacy experience or residency preferred CERTIFICATIOIN/LICENSURE ? Wisconsin/Illinois Registered Pharmacist License OTHER SKILLS AND ABILITIES ? Excellent interpretive/analytical skills for pharmaceutical applications ? Strong accuracy skills ? Computer experience WORK ENVIRONMENT ? OSHA Exposure category C SPECIAL PHYSICAL DEMANDS The noise level in the work environment is usually moderate. The employee is frequently required to stand; walk; sit and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds. Good manual dexterity to operate keyboard and syringes. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
