We are looking for an individual with a passion for patient care, dedicated to contributing to positive outcomes in a largely underserved patient population.

The ideal candidate has experience in Ambulatory Care practice, including experience managing anticoagulation and diabetes therapies, strong communication and organizational skills, and experience in precepting pharmacy learners.

Pharmacists have been embedded in ambulatory services throughout our organization over the past two decades. Our Primary Care Clinic is a Patient Centered Medical Home, and our Anticoagulation Service is recognized as an Anticoagulation Center of Excellence. We are proud to be the first in Connecticut to offer a PGY2 Ambulatory Care Pharmacy Residency Program.

    Work Hours/Shift:

  • 40hrs, Full time Mon- Fri days

    You Will Be Responsible For :

  • Strong Educator: Precepts multiple learners including IPPE, APPE, PGY1 and PGY2 Pharmacy Residents

  • Excellent Communicator : Provides medication information to medical residents, fellows and attending physicians as related to the area of practice

  • Technical Familiarity: Applies critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments

  • Exceptional Patient Care : Orders prescriptions and labs as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement

  • Active Teamwork : Identifies opportunities to expand and improve clinical pharmacy ambulatory services throughout the organization

    Requirements :

  • Education: B.S. degree in Pharmacy or Doctor of Pharmacy Degree

  • Job Knowledge/Experience: PGY2 Ambulatory Care Residency training or equivalent experience preferred

  • Licensure: Current Connecticut Pharmacist License

  • Certification: Board Certification preferred, would be required to obtain BCACP credentialing within one year of eligibility to participate in collaborative practice agreements in both Primary Care and Anticoagulation Service areas.
