Cone Health Clinical Pharmacist - Emergency Medicine in Greensboro, North Carolina

The Clinical Pharmacist would be responsible for assisting the THN Pharmacy Team on pharmacy related issues in the high risk patients involved in the THN network. The goal is to promote quality cost-effective outcomes for THN patients. The Clinical Pharmacist will meet with patients at their home, at the office, or other reasonable site. The Clinical Pharmacist will provide medication reconciliation reviews on patients referred by THN Case Managers or THN providers. The Clinical Pharmacist will check for discrepancies between the patient care summary and the patient medication list. The Pharmacist will evaluate the patient for drug related problems and will use an EMR to review patient?s conditions, laboratory test results, medications at discharge and pharmacy claims data. The pharmacist will communicate discrepancies and drug related problems found and resolutions to the discrepancy or problem with the THN provider. This care is delivered within the context of long-term health care partnership with the patient that emphasizes health improvement, wellness, and disease prevention.

The Clinical Pharmacist will also assist in evaluating the quality metrics being measured within the THN contracts.

The Clinical Pharmacist will deliver excellent customer service utilizing Relationship Based Care to advocate for safety, patient preferences, and optimal communication through partnering with patients/families and colleagues, including all internal and external customers.

Clinical Responsibilities: Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens. Participates in pharmacy activities in the care transition process to assure safe, effective, appropriate, and economical use of medications to improve continuity of care and outcomes. Participates in pharmacy activities that support the Chronic Care initiatives and other future clinical initiatives. Serves as a resource to network physicians and care managers on general drug information and Medicare pharmacy policy issues. Assists with the THN Medication Assistance Program.


Quality Management: Participates in Quality Assurance and Performance Improvement activities. Effectively applies the guidelines of the THN's performance improvement process. Serves as a representative on applicable clinical committees.


Education: Provides education and traininig to staff on processes such as Medication Reconciliation and disease state management. Serves as a resource to network physicians and care managers on general drug information and Medicare pharmacy policy issues.


Documentation: Consistently documents all activities in the Electronic Medical Record using the established format in a timely and accurate manner. Promptly sends reports to physicians and other providers as per policy and as needed to relay pertinent finding. Maintains accurate accounting of work hours and miles driven to conduct THN business activities and submits in a timely manner. Actively participates in program improvement activities.


EDUCATION:

Required: PharmD Degree Required or BS in Pharmacy with at least 5 years of clinical experience and completion of a MTM certificate program.

Preferred: Completed PGY-1 Pharmacy Practice Residency accredited by ASHP Completion of an Ambulatory Care PGY-2 Residency, BCPS certification

EXPERIENCE:

Preferred: At least five years of Pharmacy experience working in the ambulatory setting strongly preferred or a PGY-2 Ambulatory Care Residency
