Holzer Health System Acute Care Clinical Team Pharmacist in Gallipolis, Ohio
This role is responsible for coverage of open shifts in all acute care pharmacy programs at HMC-Gallipolis Campus and Cancer Center/ Infusion Center. The Acute Care Clinical Team Pharmacist will operate at a variety of facilities working in both a staffing and clinical role. This position is responsible for working with a multi-disciplinary medical care team to evaluate therapeutic assessments and disease management focusing on patient outcomes, while maintaining cost. The major duties of the Acute Care Team Pharmacist will include staffing at the HMC-G Acute Care Pharmacy and will be expected to enter, fill, and dispense medications according to all state board of pharmacy requirements. In addition, clinical duties of the Clinical Pharmacist or Pharmacy Infusion Services Supervisor will be performed in their absence as well as other duties including providing pharmaceutical information to staff pharmacist, technicians, patients and allied health partners, the education and development of students and residents.

Education, Work Experience and Licensure:
A. Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy preferred, licensure by Ohio Board of Pharmacy at time of hire required.
B. PGY-1 Pharmacy Practice Residency completion or 3 years of equivalent acute care pharmacy experience preferred.
C. Board Certification in a relevant Pharmacy Specialty (BCPS, BCCP, BCCCP, BCIDP, BCOP) within 24 months of hire required.
D. BLS and ACLS Certification required within 90 days of hire.
E. Knowledge, skills, and abilities: Extensive knowledge of pharmacy and its related sub-disciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of practice
F. Good written and verbal communication; the ability to integrate into the healthcare team
G. Proficiency in using database information systems and Microsoft Office Suite
H. Ability to research and analyze the medical literature including drug information, disease states, and clinical practice guidelines
