Work Schedule: 7:00 AM to 3:30 PM


This is a hybrid role that will split time in two separate care environments. The Clinical Pharmacist will spend 2 days per week in an Acute Care Clinical Pharmacist position at NL Mayo Hospital main campus. The remaining 3 days will be spent splitting time in NL Mayo and NL CA Dean Primary Care offices. Under the general supervision of the Directors of Pharmacy at both hospitals, the clinical pharmacist will deliver pharmaceutical care consistent with the department’s clinical role in assuring rational drug therapy in accordance with the department mission and department policies and procedures.


·Develops, evaluates, recommends, and monitors patient pharmaceutical care plan

·Assists prescribers in developing optimal drug therapies and monitors those therapies

·Performs pharmacokinetic evaluations.

·Collaborates with prescribers and caregivers to improve patient outcomes.

·Participates in development and implementation of medication pathways and protocols.

·Provides patient and family medication education.

·Ensures an accurate patient medication history.

·Helps assure continuity of pharmaceutical care through care transitions.

·Uses computer medication profiles to ensure that dosing is appropriate, and that adverse drug interactions and therapeutic duplicates or incompatibilities are avoided.

·Clarifies prescription with prescriber or caregiver as appropriate.

·Ensures safety in compounding, preparing, and dispensing of chemotherapy and other infusions for outpatient oncology clinic

·Contacts and confers with prescriber whenever prescription irregularities arise.

·Confers with prescribers and caregivers concerning availability, form, side effects, and drug interactions of medication prescribed.

·Obtains patient clinical information, performs computer order entry, and ensures appropriate product selection.

·Compounds pharmaceutical preparations as necessary, using standard pharmaceutical physical/chemical procedures and equipment.

·Participates in the department teaching, training, and competency activities.

·Precepts pharmacy student APPE and IPPE rotations.

·Prepares and presents staff, caregiver, prescriber, and community education programs.

·Documents and maintains patient and department records including, but not limited to purchasing and controlled substance records.

·Participates in departmental supply management.

·Assists in performing drug expiration date inspections/replacements.

·Assists with or may have full responsibility for special projects as assigned by the Directors of Pharmacy.

·Additional duties may include but are not limited to quality assurance/performance improvement activities, newsletter publication and committee assignments.

  • Maintain a safe environment which includes knowing and understanding hospital and departmental policies and procedures.

  • Report and directly address identified environmental hazards when appropriate.

  • Report and directly address violations of patient safety policy and/or protocol when involved or observed.

  • Other duties as requested by Directors.

Ambulatory Care Job Responsibilities

Provides comprehensive pharmaceutical care to patients within NL Mayo and NL CA Dean owned Primary Care Practices. This includes:

Reviews medications of discharged patients from local hospital or skilled nursing facility to ensure safety of high-risk populations.

Discusses medication order clarification with prescribers, documents change in patient records, and communicates changes.

Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.

Works with the primary care team and specialty providers to develop collaborative agreements to assist with disease state management.

Participates in Wellness Visits in collaboration with providers.

Supports an anti-coagulation management service.

Develops, implements, and maintains Collaborative Drug Therapy Management agreements as necessary

  • Provides guidance on immunizations and administers vaccines as needed

  • Serves as a source of clinical information for the medical and nursing staff. Responsibilities include serving as a preceptor and mentoring Pharmacy Students and providing staff development support to Physicians, Pharmacists, Nurses and support staff.

  • Assists in standardization of prescribed therapies and pharmacy processes.

  • Establishes productive, collaborative relationships with medical staff.

  • Sustains professional competence through participation in educational programs, workshops, and national/regional conferences.

  • Collaborates with other patient care team members regarding clinical interventions according to established procedures.

  • Prevents, detects, monitors, documents, and reports adverse events and medication errors.

  • Participates in projects, workgroups, and committees to improve patient outcomes.

  • Maintains appropriate documentation within the electronic health record.

Other information:

Education: BS in Pharmacy or PharmD – Graduate of an ACPE accredited College of Pharmacy

Experience: 3 years clinical pharmacy experience required

Board certification and/or PGY1 residency experience preferred

Licensure: Licensure or eligibility for licensure as a Registered Pharmacist in the State of Maine
