Overview

MercyOne Des Moines Medical Center is seeking a staffpharmacistto work inoncology. This will be a decentralized clinical position on theoncologyfloor.Pharmacistswill be working mostly days with 4 evening requirements per month and will be required to work every 4th weekend.Oncologypharmacistsat MercyOne – DSM are essential members of the interdisciplinary team who work closely withoncologistsand nurses to solve patient's pharmacotherapy problems. Oncologypharmacistsare responsible for providing pharmaceutical care tooncologypatients, including entering chemo orders and double checking chemo orders, ensuring that all patients receiving chemotherapy receive necessary supportive medications, providing drug therapy consultation, reviewing medication orders, facilitating medication distribution and patient discharges, answering drug information questions, and other tasks as necessary.

Responsibilities

  1. Delivers pharmaceutical care to patients to ensure optimal medication-related patient outcomes.

  2. Oversees and/or participates in assuring appropriate medications are prepared and delivered to care areas in a timely manner, and are stored both securely and in a manner appropriate for both stability and effective workflow.

  3. Works collaboratively with other healthcare team members to provide pharmacotherapy recommendations with intent to achieve optimal patient outcomes across the continuum of care.

  4. Supervises and mentors technicians, interns, and students working during their shift in order to maintain the accuracy and quality of pharmaceutical services.

  5. Maintains quality and safety in the department through completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensures appropriate documentation.

  6. Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and MercyOne medication use policies to assure safe and appropriate use of medications.

  7. Participates in the orientation, instruction and training ofpharmacystudents, interns,pharmacytechnicians, residents,pharmacistsand other health professionals.

  8. Knowledgeable and adheres to State and Federal laws, regulations,pharmacypolicies, procedures and standard operating procedures, formulary and pertinent clinical practice guidelines.

  9. Provides professional, engaging customer service to all patients, and healthcare providers with whom they work.

  10. Performs other job functions as directed by thepharmacymanager/supervisor.

Qualifications

  • harmacydegree (BS or Pharm.D.) from an ACPE approved college ofpharmacy(required)

  • Licensed, or eligible forpharmacistlicensure in Iowa

  • Successful completion of ASHP-accredited PGY-1PharmacyPractice Residency or equivalent experience

  • Completion of PGY-2Oncologyresidency or 3 years of experience inoncologyis preferred.

  • Board ofPharmacySpecialties (BPS) certification preferred

  • BLS within 6 months of hire
