The centers provide campus-based direct services and supports at 13 centers located throughout the state. If you are looking for a place to work where you can establish a career that is filled with purpose, this is the job for you!Provides highly specialized and complex pharmaceutical services to improve drug usage and therapeutic outcomes for persons with developmental disabilities residing at a State Supported Living Center (SSLC). Conducts Drug Utilization Evaluations (DUE)/Drug Utilization Reviews (DUR) on commonly prescribed medications at the SSLC. Analyzes and documents assessments of residents' pharmaceutical regimens at regular intervals, including issues related to use of antibiotics, anti-convulsants, psychoactive medications, and polypharmacy. Participates in Interdisciplinary Team focused on the clinical care of residents at SSLC including Health Status Team meetings and ad hoc Interdisciplinary team meetings. Provides continuing education to pharmacists, physicians, and nurses on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses. Serves on facility committees and workgroups which have a pharmacy related component. Functions independently with minimum supervision and extensive latitude for use of initiative and independent judgment within the scope of practice outlined by the Texas State Board of Pharmacy. Provide treatment to HHS employees injured in course and scope of employment.Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.Conduct quarterly medication therapy reviews for all individuals to optimize drug regimens and provide the best possible patient outcomes.
