Req# 18729Aultman Health Foundation, CANTON, OHINTEGRATED HEALTHAultman Health FoundationTemporary, Day Shift, 8-5 pm (varied days)PURPOSE OF POSITIONThe Integrated Health Collaborative (IHC) Temporary Pharmacist Intern or Pharmacist works collaboratively with IHC leadership, IHC member practices, hospitals, and staff to enable improved quality, cost and satisfaction for contracted beneficiaries. This collaborative work may include performing various evidence-based, patient-centered, cost-effective, clinical, educational, and informative functions to understand opportunities for improvement, develop and implement action plans, and work directly in clinical settings to help support patients, practitioners, and staff. The Temporary Pharmacist Intern or Pharmacist uses a combination of proactive telephone calls, hospital and office visits, and mailings to contracted beneficiaries who are recognized as needing pharmacy intervention and assistance. RESPONSIBILITIES & EXPECTATIONSProvides pharmaceutical expertise to the IHC leadership to improve the health of the populations and achieve success in IHC value-based initiatives.Proactively assesses opportunities for medication-related cost utilization, medication adherence, pharmacy related contracted measures, and comprehensive medication reviews.Demonstrates ability to interpret data related to pharmaceutical utilization and make recommendations for improvement based on the data.Displays emphasis on improving the quality of care and reducing cost. Communicates with and educates member providers, hospital staff and leadership. Works with patients and providers to contribute to care plan, promoting adherence to care plan.Establishes a follow-up schedule and monitors patients' progress related to pharmacy interventions.Connects patients and families to community resources related to pharmaceutical interventions.Provides education, serving as a drug information resource. For select patients, monitors drug therapy and evaluates dose, form, regimen, duplications, drug interactions, and optimization.Effectively and accurately uses Electronic Health Records (EHR) as necessary to document and track patient history.Clearly and effectively communicates with the patient and care team.Other duties as assigned.KNOWLEDGE, SKILLS AND ABILITIES:Demonstrates responsibilities and expectations of job.Demonstrates strong interpersonal skills.Displays comfort with patient-centered approach that allows the patients' desires around behavior change to inform the plan of care.Displays clinical skills or aptitude in problem-solving and strategies for behavior change, including Motivational Interviewing.Ability and willingness to work in a time-limited, structured, and solution-focused environment.Ability and willingness to function proactively in various health care settings.Ability to maintain a professional stance if conflicts arise.Flexibility to adapt to unforeseen needs or circumstances.Knowledge of computer systems as below.TRAINING AND EXPERIENCE REQUIREMENTS: Pharm.D. degree required.Must be in good standing with the Ohio Board of Pharmacy.Eligible for licensure or Licensed as a pharmacist in Ohio is required.Maintains competency in aspects of drug therapy for ambulatory care patients.Ability to advocate for patients and families.Must be able to communicate effectively, timely, and in an open, honest manner.Knowledge of computer systems (Microsoft Word, Microsoft Excel, presentation software, EHR systems).
