I.Position Summary:

a.Helps achieve the vision of MacNeal Hospital pharmacy services by ensuring that staff duties are executed in a customer focused, cost effective and results oriented manner

Engages in multidisciplinary rounding as indicated. Identifies actual and potential medication related problems through review of medication history, patient profile, medical records and interviews with patients and caregivers. B. Provides detailed therapeutic consultation, drug information, education, and literature reviews to medical, nursing, pharmacy and other allied health care professionals. C. Initiates patient medication education and medication reconciliation activities when needed

Provides education to physicians, nurses, and other health care professionals, including inservices and lectures. Answers questions and provides guidance for clinical activities of staff pharmacists

Participates in the activities of various committees, including, but not limited to: preparation of formulary reviews, drug monographs, policies and procedures

Clinical pharmacists provide comprehensive pharmaceutical care to designated service areas or patient populations to achieve optimal patient outcomes and safety. They function as members of multidisciplinary teams to improve medication therapy management. These positions act as drug information resources and their responsibilities include age-specific and/or disease specific consultative medication therapy evaluation, education, research and participation in quality improvement activities.

Minimum Education:

Required: Bachelors Degree

Preferred: Doctor of Pharmacy (PharmD)

Residency: Pharmacy Practice - General (Preferred), Residency - Specialty(Preferred)

Required: 1-2 years of previous job-related experience
