Mission Health Pharmacist Transitions in Care in Asheville, North Carolina

Job Summary: Provides quality transitional care and cost-effective pharmacotherapeutic management to improve patient outcomes at Mission Hospital. Provides pharmacotherapeutic and didactic instruction to UNC Eshelman School of Pharmacy students by developing a transitional care clerkship at Mission Hospital. Provides supervision of pharmacy technicians engaged in the provision of transitional care activities.

Required Education: Completion of a Doctor of Pharmacy degree from an accredited school or college of pharmacy. Completion of a PGY1 residency or completion of Board Certification in Pharmacotherapy or Ambulatory Care

Required License: Maintenance of a current and active license to practice pharmacy in North Carolina or current eligibility to become licensed in North Carolina within 60 days of hire required

Preferred License: Completion of Board Certification in Pharmacotherapy or Ambulatory Care. Current eligibility for a Clinical Pharmacist Practitioner (CPP) license

Preferred Experience: Demonstration of healthcare resource navigation skills in Western North Carolina. Demonstration of effective precepting skills with pharmacy students and residents. Demonstration of effective supervisory skills with pharmacy technicians. Completion of two or more years of direct patient care experience Demonstration of skills in leadership and scholarship Demonstration of prior experience or training using Cerner. Demonstration of effective written and verbal communication skills with patients and with an interdisciplinary team Demonstrated ability to multi-task and prioritize
